Prior to scheduling an appointment, you will need to submit a consultation request form by selecting the "Request Consultation" tab at the page menu. After your request is reviewed by the  psychiatrist,  you will receive a link to our patient portal: SimplePractice. In SimplePractice, you can self-schedule appointments based on availability, complete additional forms, reach the provider directly via secure messaging feature, upload and download records etc.
